Please note this form is not a confirmation of your appointment, Denise will be in contact with you to confirm the closest appointment time available to suit your request. If you are a new client it is recommended that when booking your initial appointment you also book in your follow up appointment/s due to our 3-6 week wait times.  We look forward to being part of your healing journey. Cancellation Policy

By The Bay Kinesiology sends an appointment confirmation SMS message the week of your appointment. Please note once your appointment has been confirmed, if cancelled with less than 24 hours notice or a ‘no show’ a cancellation fee of 50% of the consultation fee will apply.  When given enough cancellation notice, I am then able to contact client’s on my waiting list to offer them the opportunity to have a kinesiology session in your place. Thank you for your understanding.

Payment Methods

Full payment of your kinesiology session is due at the end of of your appointment. By The Bay Kinesiology accepts payment via cash, electronic funds transfer to our nominated bank account and Visa or Mastercard credit/debit cards. Please note a 1% bank card processing fee is incurred with all Visa and Mastercard payments.  There are no EFTPOS facilities available. On request a receipt can be emailed to you for your records.

Gift Vouchers Are Available

Gift Vouchers are a wonderful present for your loved ones.  Gift vouchers are available for use within 12 months from date of issue and subject to appointment availability.  To avoid disappointment, please book appointments at least two weeks in advance.

Health Fund Rebate

You may be able to obtain a Health Fund Rebate on Kinesiology or Touch for Health Kinesiology services through your Health Insurance Fund Provider for treatments provided by Denise Robinson.  Only some Health Funds cover Kinesiology and it depends on the type of extras cover you have as to whether you can obtain a health refund benefit.  Please contact your health insurance provider to find out if you are eligible.  Some known participating funds include:  Australian Health Management; Australian Unity Health Ltd; CUA Health; CBHS Health Fund Limited; Grand United Corporate; Health Care Insurance (HCI); Manchester Unity; MBF Alliances; Medibank Private; Teachers Federation; Westfund Health; Health.com.au.  Please be advised BUPA does not cover our services.
